As physicians, we strive for evidence-based medical practices to achieve the best patient care outcomes. For the past period of time, social determinants have been overlooked and not considered when structuring a healthcare plan for different patients. Since we are privileged to serve a vulnerable population, I believe that individualized healthcare approaches could be a step in the right direction to reduce emergency department visits and unnecessary admissions, where establishing a primary care relationship could be the long-term alternative. Another step would be to assemble teams consisting of physicians, social workers and pharmacists to populate effective plans with adequate costs and a support system for patients. Patient care is the primary concern for healthcare professionals and the ultimate goal of our career where social determinants are proven to play a vital part and should be integrated into our daily medical practice.
