Last Friday i saw a new pt who was discharged recently from DRH to SAR after being diagnosed with L MCA stroke, pt was d/s’ed from SAR, pt has no home, his vision is so bad to the level that he need to be guided for walking, pt currently living with a friend who brought him in today, medical condition been taken care of, me & my preceptor spent a lot of time working on his future plans, worked with our nurse so we can arrange a placement for a NH & hoping he will be accepted in one of these homes in order to avoid being a homeless pt, pt also was refereed to Rosa park geriatric clinic for more resources.
