I had a 63 yo female who follows up with me very closely with multiple medical issues and depression. Her depression was worsening and so was generalized pain and acid reflux. I tried to get to the root cause of her worsening symptoms and found out that she was given urgent notice to evict from her house and she was very stressed that pt will become homeless soon. I tried to counsel her regarding her stress and depression and other chief complaints, but one thing which could have helped was a social worker in-house who would have helped her with her ongoing housing issues. I believe the best way to approach pt’s issues is working as a multidisciplinary team. We as a physician, even by going above and beyond what we can do is very limited and this affects the social determinants in a huge way.
